◈ Contact Person's Name & Email Address
◈ Copy of Business License
◈ Tax Identification Number. - Once your company information is confirmed, one of our dedicated Account Representatives will email you to create an order with us.
- Within 1-2 business days, you will receive a PDF invoice via email if your order meets our minimum requirements along with an ETA on the order. Orders not meeting our criteria will be cancelled.
- Upon receipt of the invoice, print a copy and mail it to us along with your payment via check or money.
